We are looking for an upbeat Patient Service Representative who can support our Clinic in day-to-day operations. Our Patient Service Representative plays a critical role in our team’s success. As the first person who greets our patients when they enter our clinic and the last person who wishes them well as they leave, you are the person patients remember when they think of our company.
We're looking for someone who can create a great patient experience (from the moment patients enter the clinic to when they leave) and carry out our front-office processes promptly and consistently.
The Patient Service Representative is responsible for working in the front office. This individual provides patient support and communication by facilitating an exceptional patient experience to build and reinforce satisfaction and trust and drive organizational loyalty. Accountable for welcoming and preparing the patient for their appointment, the patient service representative informs patients of relevant and required information for their visit and provides clear communication around the services they are scheduled to receive.
Expectation:
- Greets all patients and visitors in a professional and welcoming manner
- Obtains copies of insurance cards, driver's licenses, authorizations, referrals, and other required appointment documentation and appropriately saves them in practice EMR
- Performs demographic and insurance validation and informs patients of privacy policies and procedures
- Effectively communicates unexpected schedule delays to patients and assists with patient comfort should delays arise
- Collects patient co-payments, reconciliation charges, and outstanding balances upon Check-In, as well as submits batches daily
- Be honest and trustworthy
- Keeps the reception and patient waiting areas clean and organized
- Schedules patient appointments/follow-up appointments per established policies and procedures

Experience:
- High School Diploma or GED required
- Detail-oriented, reliable, and able to multi-task in a fast-paced, high-volume work environment

- Medical Front Desk Experience preferred; experience in primary care or family practice preferred
- Flexible schedule working Monday-Friday and some Saturdays

Physician
Family Medicine or Internal Medicine
Provides comprehensive medical services, regardless of age or sex within scope of training. Services include preventative care, immunizations, primary care and treatment, health education, prescribing of medications, ordering and interpreting lab testing, physical examinations, health maintenance screenings, vision/hearing, women’s health, family planning, and nutrition education. Occasional evening and Saturday shifts. Outpatient only. No hospital call or rounds. May be asked to supervise and collaborate with APCs. May be asked to provide training to resident physicians or physician extender students. Interest in treating and working with persons living with HIV and underserved populations in a Ryan White funded clinic.
REQUIRED SKILLS AND QUALIFICATIONS
Education:
- Medical Degree (MD, DO)
- Family Practice or Internal Medicine Residency training
Experience:
- Within scope of training
- Familiar with Electronic Health Records (EHR), basic computer skills expected
Licensure/Certification: as appropriate
- MI Medical License, DEA Certificate
- BLS certified
- Board certified or Board eligible within three years of residency training
- Interest in becoming HIV certified with AAHIVM within 3 years of hire date

Medical Case Manager
The Case Manager (CM) provides case management services in either the community and/or in-office setting to patients and their families. The CM assists the patient and care team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress. The CM serves as a patient advocate and liaison between the patient/family, medical provider(s), and community organizations by identifying community resources to improve or enhance the patient's quality of life. The position will effectively collaborate with community organizations. The position creates connections between vulnerable populations, the health care system, and community resources. The individual will provide patient outreach and resource navigation for individuals served, culturally appropriate health education and information, and give informal counseling and guidance on health behaviors. Experience working with persons living with HIV and underserved populations is a plus.
Responsibilities:
- Coordinates patient care with the Care Team, as well as other facilities and staff.
- Assists in the development, implementation, and revision of individual treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals.
- Works in various settings with culturally diverse individuals and communities with the ability to be culturally sensitive and appropriate.
- Assists in completing patients' closures.
- Share knowledge and experience with patients, as appropriate.
- Advocate for patients and their communities.
- Facilitate two-way communication between patients and the Care team.
- Coordination of care with external partners and Medical providers.
- Outreach and engagement for patients assigned to the site, all patients who miss scheduled sessions or appointments.
- Ensures family members and other natural supports identified by the patient have releases of information on file and are involved in the patient's ongoing treatment.
- Participates in case of staffing with other care teams as necessary.
- Writes reports, business correspondence, or procedures manuals.
- Assists patients in identifying community resources to assist in their healthcare and encourages patients to attend appointments, support groups, or other services to maintain ongoing optimal health and build outside support.
- Demonstrates knowledge of relevant federal and state regulations on funding requirements.
- Attends scheduled meetings and participation in the necessary training
- Works in the areas of community outreach/marketing, promoting services
REQUIRED SKILLS AND QUALIFICATIONS
Education:
- Bachelor’s degree in a related field, Masters a plus
Experience:
- 2 years experience, preferably in a health-related area
- Outreach in a community setting, including experience with public presenting (small groups) Current MI Driver’s License
Expectations:
- Background Check cleared
- Good communication skills.
- Ability to work as a team member.
- Competent computer keyboard, laptop maneuvering, competent in the Windows platform
- Basic computer skills and database entry.
- Good organizational skills.
- Ability to handle difficult calls and maintain professional conduct.
- Willingness to be flexible with job responsibilities
- Passion for and dedication to working with community members
- Excellent phone mannerisms
- Ability to effectively engage in conversation, motivate and engage patients to participate in our program, and speak to all types of patients.
- Flexible daytime hours to stretch to 6 pm M-F and some Saturdays if needed to reach pts after work hours.
- Will be expected to attend training and continuing education credits.
